- To ensure your best experience, we ask that you come to your initial appointment with your new patient documents complete, or arrive 25 minutes early to your initial appointment.
- Please bring your driver’s license or identification card and your insurance card, if we are planning to bill your insurance.
- Be sure to wear comfortable clothing
- Eat something small before your appointment.
- Please let us know if there is anything we can do to make your experience more comfortable.

These forms will be emailed to you through our secure Jane App portal. Please use the links in the email to access and complete them online. Filling out these forms ahead of time helps us prepare for your visit and ensures a smooth start to your care.

Please feel free to contact us to check your specific insurance company and plan benefits.
In addition to billing private insurance, we also bill motor vehicle insurance or car insurance.
We offer a time-of-service discount for patients who are not billing insurance.
